Purpose & Extent of Information:
USA Elite Training, LLC is creating a Hall of Fame. The Hall of Fame will consist of former and current students of USA Elite Training, LLC who have excelled in the sport of softball. To accomplish this, USA Elite Training, LLC is dedicating a section of our web page, listing player’s achievements. Information that will be included is as follows:
- First Name
- Last Name
- High School
- Team Name (College, high school, travel)
- Year of Graduation
- College, High School, travel softball, and Little league/youth softball achievements and statistics and awards

By completing the Hall of Fame information Form on line with USA Elite Training, LLC, you represent and warrant that:
a. All information you have posted is accurate; and
b. Your posted of the information does not violate any applicable law or the rights of any other party.
